Дегенеративный стеноз поясничного отдела позвоночника – распространенная причина хронического болевого синдрома и нейрогенной перемежающейся хромоты у пациентов пожилого и старческого возраста. Клинические проявления часто не коррелируют со степенью сужения позвоночного канала по данным магнитно-резонансной томографии, что требует применения комплексных методов диагностики, включая электрофизиологические.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: провести комплексный анализ взаимосвязей между данными магнитно-резонансной томографии, параметрами электронейромиографии и выраженностью клинической симптоматики у пациентов с дегенеративным стенозом поясничного отдела позвоночника.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Комплексное клинико-инструментальное исследование проведено 71 пациенту в возрасте от 60 до 82 лет с диагнозом «дегенеративный стеноз поясничного отдела позвоночника». Оценивалась боль по визуально-аналоговой шкале, функциональный статус по шкале Освестри и площадь дурального мешка по данным магнитно-резонансной томографии. Электронейромиографическое исследование включало оценку Н-рефлекса с малоберцового и большеберцового нервов. Статистический анализ выполнялся с использованием коэффициента корреляции Спирмена.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена умеренная отрицательная корреляция между площадью дурального мешка и латентностью Н-рефлекса (r = -0,68, p&lt;0,05). Связь между степенью стеноза и интенсивностью боли по визуально-аналоговой шкале была слабой и незначимой (r=0,32, p&gt;0,05). Наибольшая корреляция отмечена между увеличением латентности Н-рефлекса и ухудшением функционального статуса по индексу Освестри (r = -0,81, p&lt;0,01).</p></sec><sec><title>Заключение</title><p>Заключение. Электрофизиологические методы, в частности исследование Н-рефлекса, являются высокочувствительным инструментом для объективной оценки функционального состояния корешков и нервов спинного мозга при дегенеративном стенозе позвоночного канала. Показатели электронейромиографии демонстрируют более тесную корреляцию с функциональным статусом пациента, чем с данными нейровизуализации, что делает их ценным инструментом для мониторинга и обоснования тактики лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Degenerative lumbar spinal stenosis is a common cause of chronic pain and neurogenic intermittent claudication in elderly and senile patients. Clinical manifestations often do not correlate with the degree of spinal canal narrowing as determined by magnetic resonance imaging, requiring the use of comprehensive diagnostic methods, including electrophysiological ones.</p></sec><sec><title>Aim</title><p>Aim: was to conduct a comprehensive analysis of the relationships between magnetic resonance imaging data, electroneuromyography parameters, and the severity of clinical symptoms in patients with degenerative lumbar spinal stenosis.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A comprehensive clinical and instrumental study was conducted on 71 patients, aged 60 to 82 years, diagnosed with degenerative lumbar spinal stenosis. Pain was assessed using a visual analog scale, functional status using the Oswestry scale, and dural sac area using magnetic resonance imaging. The electroneuromyography study included an assessment of the H-reflex from the peroneal and tibial nerves. Statistical analysis was performed using the Spearman correlation coefficient.</p></sec><sec><title>Results</title><p>Results. A moderate negative correlation was found between the dural sac area and the H-reflex latency (r = -0.68, p &lt; 0.05). The relationship between the degree of stenosis and pain intensity according to the Visual Analogue Scale was weak and insignificant (r = 0.32, p &gt; 0.05). The highest correlation was observed between an increase in the H-reflex latency and a deterioration in the functional status according to the Oswestry index (r = -0.81, p &lt; 0.01).</p></sec><sec><title>Conclusion</title><p>Conclusion. Electrophysiological methods, particularly H-reflex testing, are a highly sensitive tool for objectively assessing the functional state of spinal roots and nerves in degenerative spinal stenosis.
