Одной из самых распространенных в клинической практике аритмий является фибрилляция предсердий. Восстановление синусового ритма предпочтительно при данном виде аритмии. Проведение операции радиочастотной аблации способствует улучшению прогноза и качества жизни пациента. Одномоментная коронарная реваскуляризация при персистирующей фибрилляции предсердий позволяет добиться ожидаемого результата. Однако, одномоментное проведение этих операций способно увеличить риск послеоперационных осложнений, в том числе и рецидива аритмии. Амбулаторное ведение таких пациентов затруднено, так как нет четких схем антиаритмической терапии в зависимости от коморбидного статуса пациента.</p></sec><sec><title>Клиническое наблюдение</title><p>Клиническое наблюдение. В данной статье представлен клинический случай послеоперационного ведения пациента 62 лет после радиочастотной аблации аритмогенных зон и резекции ушка левого предсердия при персистирующей фибрилляции предсердий, совмещенной с операцией коронарного шунтирования с помощью одноканального монитора электрокардиограммы в чехле смартфона.</p></sec><sec><title>Заключение</title><p>Заключение. Дистанционный телеметрический контроль электрокардиограммы в чехле смартфона CardioQVARK прост в использовании, позволяет получить дополнительную информацию о работе сердечно-сосудистой системы. Ранее выявление рецидивов аритмии позволяет врачу оптимизировать проводимую антиаритмическую терапию, улучшить прогноз заболевания и качество жизни пациентов. Использование современного неинвазивного телеметрического контроля электрокардиограммы является перспективным направлением развития кардиологии. Оно может улучшить качество медицинской помощи и повысить эффективность лечения пациентов с сердечно-сосудистыми заболеваниями.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. One of the most common arrhythmias in clinical practice is atrial fibrillation. Restoration of sinus rhythm is preferable for this type of arrhythmia. Radiofrequency ablation improves the prognosis and quality of life of the patient. Simultaneous coronary revascularization in persistent atrial fibrillation allows achieving the expected result. However, the simultaneous performance of these operations can increase the risk of postoperative complications, including recurrence of arrhythmia. Outpatient management of such patients is difficult, since there are no clear antiarrhythmic therapy regimens depending on the patient's comorbid status.</p></sec><sec><title>Case report</title><p>Case report. This article presents a clinical case of postoperative management of a 62-year-old patient after radiofrequency ablation of arrhythmogenic zones and resection of the left atrial appendage for persistent atrial fibrillation, combined with coronary artery bypass grafting using a single-channel electrocardiogram monitor in a smartphone case.</p></sec><sec><title>Conclusion</title><p>Conclusion. Remote telemetric monitoring of the electrocardiogram in the CardioQVARK smartphone case is easy to use and allows you to obtain additional information about the functioning of the cardiovascular system. Early detection of arrhythmia recurrence allows the doctor to optimize the antiarrhythmic therapy, improve the prognosis of the disease and the quality of life of patients. The use of modern non-invasive telemetric monitoring of the electrocardiogram is a promising direction in the development of medicine.
