Инфаркт миокарда без обструктивных поражений коронарных артерий определяется наличием признаков инфаркта миокарда при ангиографических данных о нормальных или почти нормальных коронарных артериях. Инфаркт миокарда без обструктивных поражений коронарных артерий может быть инфарктом миокарда 1-го или 2-го типа. Гипертрофическая кардиомиопатия является возможной причиной инфаркта миокарда без обструктивных поражений коронарных артерий.</p></sec><sec><title>Цель</title><p>Цель. Представить возможности диагностики инфаркта миокарда и определения его типа у больного с гипертрофической кардиомиопатией.</p><p>Описание клинического случая. Описан клинический случай инфаркта миокарда без обструктивных поражений коронарных артерий у мужчины с гипертрофической кардиомиопатией. 57-летний мужчина обратился с жалобами на загрудинную боль, которая появилась в покое, во время приступа тахиаритмии. На электрокардиограмме выявлена блокада правой ножки пучка Гиса и инверсия зубца T в отведениях I, avL, V1-V6. Нарастание уровня сердечного тропонина T с последующим снижением свидетельствовало о инфаркте миокарда. Эхокардиография выявила асимметричную гипертрофию межжелудочковой перегородки с толщиной базального и среднего сегментов перегородки 18 мм, передне-систолическое движение передней створки митрального клапана и обструкцию выходного тракта ЛЖ с пиковым градиентом систолического давления 50 мм рт. ст., диастолическую дисфункцию левого желудочка. При проведении коронарографии обнаружен стеноз среднего сегмента левой передней нисходящей артерии менее чем на 50 %. Все эти данные позволяют диагностировать инфаркт миокарда без обструктивных поражений коронарных артерий. В связи с наличием гипертрофической кардиомиопатии наиболее вероятен инфаркт 2 типа, однако отсутствие внутрикоронарной визуализации не позволяет полностью исключить 1 тип инфаркта миокарда.</p></sec><sec><title>Заключение</title><p>Заключение. Клинический случай демонстрирует развитие инфаркта миокарда без обструктивных поражений коронарных артерий при гипертрофической кардиомиопатии. Для исключения инфаркта миокарда 1 типа необходима внутрикоронарная визуализация.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Myocardial infarction with non-obstructive coronary arteries is defined by the presence of clinical symptoms and signs of myocardial infarction despite angiographic findings of normal or near-normal coronary arteries. Myocardial infarction with non-obstructive coronary arteries can be classified into myocardial infarction type 1 or type 2. Hypertrophic cardiomyopathy is a possible underlying etiology of myocardial infarction with non-obstructive coronary arteries.</p></sec><sec><title>The aim</title><p>The aim. To present the diagnosis of myocardial infarction in a patient with hypertrophic cardiomyopathy.</p></sec><sec><title>Case descriptions</title><p>Case descriptions. A clinical case of myocardial infarction with non-obstructive coronary arteries in a man with hypertrophic cardiomyopathy is presented. 57-year-old man presented with retrosternal chest pain that appeared at rest but during an attack of tachyarrhythmia. Electrocardiogram showed right bundle branch block and T inversion in the I, avL, V1-V6. Cardiac troponin T was with a rise and fall in the level. Echocardiography demonstrated asymmetrical septal hypertrophy with thickness of the basal and medium segments of the septum 18 mm, systolic anterior motion of the mitral valve, and left ventricular outflow tract obstruction with a peak systolic pressure gradient of 50 mmHg, diastolic dysfunction of the left ventricle. Coronary angiography revealed stenosis of the left anterior descending artery &lt; 50 %. All these data allow us to diagnose the myocardial infarction with non-obstructive coronary arteries.  Type 2 infarction is most likely, however, the absence of an intracoronary imaging does not completely exclude type 1 myocardial infarction.</p></sec><sec><title>Conclusion</title><p>Conclusion. The clinical case demonstrates myocardial infarction in the absence of obstructive coronary artery disease in patient with hypertrophic cardiomyopathy.
