Пациенты с длительно текущей дискогенной болью в нижней части спины, у которых не наблюдается клинического улучшения после консервативного лечения, направляются на хирургические методы лечения, такие как поясничный спондилодез или тотальная артропластика межпозвонковых дисков.</p><p>Цель исследования – оценить долгосрочную эффективность и безопасность тотальной артропластики межпозвонковых дисков и поясничного спондилодеза у пациентов с дегенеративным заболеванием поясничных межпозвонковых дисков.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Разработана комплексная стратегия поиска литературных источников в базах данных PubMed/Medline и Кокрейновского центрального регистра рандомизированных контролируемых исследований. Поиск исследований проводился в период с января 2005 года по май 2024 год без ограничений по уровню доказательности или статусу публикаций. Анализировались как англоязычные статьи, так и отечественные публикации.</p></sec><sec><title>Результаты</title><p>Результаты. В рамках анализа соответствия систематическому обзору включены четыре рандомизированных контролируемых исследования, охватывающих долгосрочные результаты хирургического лечения 1325 пациентов с дегенеративным заболеванием поясничных межпозвонковых дисков. Различия в значениях продолжительности операции в группах тотальной артропластики и поясничного спондилодеза отсутствуют. В группе, где выполнялся поясничный спондилодез, объем кровопотери превысил таковой параметр в группе тотальной артропластики межпозвонковых дисков. Отсутствуют различия между группами тотальной артропластики и поясничного спондилодеза в степени выраженности боли в нижней части спины по визуальной аналоговой шкале. В группе тотальной артропластики поясничных межпозвонковых дисков уровень дееспособности пациентов по Oswestry Disability Index в среднем на 9 % выше, чем в группе поясничного спондилодеза. Процент удовлетворенности операцией в группе пациентов после тотальной артропластики превышает данный показатель в группе пациентов, перенесших поясничного спондилодеза. Проведенный анализ частоты встречаемости осложнений продемонстрировал отсутствие различий между сравниваемыми методиками. Частота выполнения реопераций в группе тотальной артропластики межпозвонковых дисков на пояснично-крестцовом уровне на 48 % меньше такового показателя в группе поясничного спондилодеза.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные клинические результаты тотальной артропластики межпозвонковых дисков на поясничном уровне с сохранением амплитуды движений оперированного сегмента и всего пояснично-крестцового отдела позвоночника позволяют утверждать, что данная методика является перспективной альтернативой поясничного спондилодеза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Patients with long-standing discogenic low back pain who do not show clinical improvement after conservative treatment are referred for surgical treatments such as lumbar fusion or total disc arthroplasty.</p><p>The purpose –– to evaluate the long-term effectiveness and safety of total intervertebral disc arthroplasty (TDA) and lumbar interbody fusion (LIF) in patients with degenerative lumbar intervertebral disk disease.</p></sec><sec><title>Material and methods</title><p>Material and methods. A comprehensive literature search strategy was developed in the PubMed/Medline and Cochrane Central Register of Randomized Controlled Trials databases. Studies were searched between January 2005 and May 2024 without restrictions on level of evidence or publication status. Both English-language articles and domestic publications were analyzed.</p></sec><sec><title>Results</title><p>Results. The systematic review included four randomized controlled trials covering the long-term outcomes of surgical treatment of 1325 patients with degenerative lumbar intervertebral disk disease. There are no differences in the operating time in the total intervertebral disc arthroplasty and lumbar interbody fusion groups. In the lumbar interbody fusion group, the volume of blood loss exceeded that in the total intervertebral disc arthroplasty group of intervertebral disks. There were no differences between the total intervertebral disc arthroplasty and lumbar interbody fusion groups in the severity of low back pain on the visual analogue scale. In the total intervertebral disc arthroplasty group, the level of patient disability according to the Oswestry Disability Index is on average 9 % higher than in the lumbar interbody fusion group. The percentage of satisfaction with the operation in the group of patients after total intervertebral disc arthroplasty exceeds this indicator in the group of patients who underwent lumbar interbody fusion. The analysis of the incidence of complications demonstrated the absence of differences between the compared techniques. The frequency of reoperations in the total intervertebral disc arthroplasty group of intervertebral disks at the lumbar level is 48 % less than that in the lumbar interbody fusion group.</p></sec><sec><title>Conclusion</title><p>Conclusion.
