Регулярная двигательная активность – это один из ключевых принципов здорового образа жизни, который доказано продлевает жизнь, как здоровым, так и лицам с сердечно-сосудистыми заболеваниями. Но появляется немало данных о потенциально возможных неблагоприятных эффектах интенсивных физических нагрузок, необходимых для участия в профессиональном или любительском спорте.</p><p>Описание клинического случая. В работе описан клинический случай выраженного коронарного атеросклероза у спортсмена зрелого возраста. Приведены клинические, лабораторные, инструментальные данные пациента, а также подробная тактика ведения с учетом современного состояния вопроса. Обсуждаются возможные механизмы повышенной кальцификации коронарного русла у лиц, вовлеченных в интенсивный тренинг.</p></sec><sec><title>Заключение</title><p>Заключение. Необходимо понимать особенности течения ишемической болезни сердца у спортсменов старших возрастных групп: высокая толерантность к физической нагрузке, нередко субклиническое течение заболевания, что подразумевает более активный распрос и использование нагрузочных визуализирующих методик. Малосимптомный характер клинических проявлений, особенности гемодинамики требуют внимательного выбора антиангинальной терапии. Лечение ишемической болезни сердца у данной когорты лиц должно проводиться соответственно действующим клиническим рекомендациям, при этом рекомендации врача, касающиеся интенсивности физической нагрузки, должны быть строго индивидуальными и основываться на результатах нагрузочных тестов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Regular physical activity is one of the key principles of a healthy lifestyle, which has been proven to prolong life in both healthy people and people with cardiovascular diseases. However, there is a lot of data on the potential adverse effects of intense physical activity required to participate in professional or amateur sports.</p></sec><sec><title>Case descriptions</title><p>Case descriptions. The work describes a clinical case of severe coronary atherosclerosis of a mature-aged athlete. The clinical, laboratory, and instrumental data of the patient are presented, as well as detailed management tactics taking into account the current state of the issue. Possible mechanisms of increased calcification of the coronary bed in people involved in intensive training are discussed.</p></sec><sec><title>Conclusion</title><p>Conclusion. It is necessary to understand the features of the course of coronary heart disease in athletes of older age groups: high tolerance to physical activity, often subclinical course of the disease, which implies more active questioning and the use of stress visualization techniques. Low-symptom nature of clinical manifestations, hemodynamic features require careful selection of antianginal therapy.
