Септопластика - одна из наиболее востребованных хирургических операций в оториноларингологии и пластической хирургии.</p><p>Хирургическая техника выполнения операции широко вариативна: возможны эндоскопические, эндоназальные и открытые операции. Методы устранения перегородки носа могут быть разделены на две категории: септэктомия по Killian и септопластика по Cottle. У септопластики по Cottle два основных приемущества: сохраняется большая порция перегородки и не требуется двусторонняя отсепаровка слизистой. Благодаря этому снижается риск опущения кончика носа, развития седловидной деформации и гематомы перегородки. Данная хирургическая техника приводит к типичным осложнениям, одним из наиболее распространенных является обильное кровотечение. У пациентов с комбинированной септопластикой и турбинопластикой значительно чаще встречаются гематома перегородки, гипосмия, длительное заживление из-за инфекции, спайки.</p><p>Целью исследования был анализ развития осложнений после выполнения септопластики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнен ретроспективный анализ клинических случаев после септопластики за период времени с 2022 по 2023 гг. в городе Иркутске. За указанный период критериям исследования соответствовали 34 пациента. Хирургическое лечение включало эндоназальную септопластику с турбинопластикой.</p></sec><sec><title>Результаты</title><p>Результаты. При построении логистического анализа значимым предиктором кровотечения установлена продолжительность операции более 100 минут (ОШ 53,9; 95 % ДИ 1,02; 941,0; р = 0,049). Получена модель с хорошей прогностической ценностью (AUC = 0,73). Чувствительность и специфичность теста составили 50 % и 96,88 % соответственно. Общий уровень достоверности – 94,12 %.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные сведения о предикторах послеоперационного кровотечения после выполнения септопластики позволяют прогнозировать негативные исходы и предупреждать их развитие с помощью активных гемостатических процедур. Использование транексамовой кислоты для пациентов групп риска по послеоперационному кровотечению позволит снизить вероятность данного осложнения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Septoplasty is one of the most popular surgical operations in otolaryngology and plastic surgery.</p><p>The surgical technique of performing the operation is widely variable: endoscopic, endonasal and open operations are possible. Nasal septum repair methods can be divided into two categories: Killian septectomy and Cottle septoplasty. This surgical technique leads to typical complications, one of the most common being heavy bleeding. In patients with combined septoplasty and turbinoplasty, septal hematoma, hyposmia, prolonged healing due to infection, and adhesions are much more common.</p><p>The aim of the study was to analyze the development of complications after septoplasty.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective analysis of clinical cases after septoplasty was performed for the period from 2022 to 2023 in the city of Irkutsk, Russian Federation. During this period, 34 patients met the study criteria. Surgical treatment included endonasal septoplasty with turbinoplasty.</p></sec><sec><title>Results</title><p>Results. When constructing a logistic analysis, a significant predictor of bleeding was found to include an operation duration of more than 100 minutes (OR 53.9; 95 % CI 1.02; 941.0; p = 0.049). A model with good predictive value was obtained (AUC = 0.73). The sensitivity and specificity of the test are 50 % and 96.88 %, respectively. The overall confidence level is 94.12 %.</p></sec><sec><title>Conclusion</title><p>Conclusion. The information obtained about the predictors of postoperative bleeding after septoplasty allows us to predict these negative outcomes and prevent their development using active hemostatic procedures.
